You can fill the injury report form out online by clicking the link below or obtain a paper copy from the Activities Office.
Student Injury Report Form
Who completes the form:
- If on site, Beth Swanson completes an injury report form (within 24 hours) for the athlete.
- If Beth is not on site, the coach completes the injury report (within 24 hours) for the athlete.
Completed injury report will be sent to Scott Bergman (AD), Beth Swanson (Athletic Trainer), and Angie Christians (AD Asst).
